Indeed, the quadruple ko only happens once in every hundred years.
But from 2000 to 2012, just less than 13 years, as I know there are 4 professional games ended in quadruple ko! They are: Hasegawa Sunao vs Doi Makoto in 2000, Qiu Jun vs Yu Bin in 2002, Chang Hao vs Lee Changho in 2005, Lee Sedol vs Gu Li in 2012.
